DAY FIELD TRIP PERMISSION FORM AND WAIVER
 
Your child has received charter school approval to participate in a field trip.  Under the California Educational Code and Board Policy, teachers and support staff may take students on field trips to enrich and complement their educational experience.  Such trips are always under the supervision of at least one teacher and/or school administrator, or certified athletic coach in the case of a same day athletic event, and all precautions are taken to ensure each student’s welfare.

WISH Community Middle School
7400 W. Manchester Ave, Los Angeles, CA 90045

Students, teachers, and chaperones will be taking a bus to and from the UCLA campus.

Important Regisration dates

**The PERMISSION SLIP and PAYMENT must be completed by Monday, January 12, 2026

**The LUNCH FORM must be completed by Monday, January 12, 2026

If, for any reason, you are unable to cover the cost for the trip at this time, please reach out to Mr. Gray, so he can make arrangements to ensure your student can attend.
